Even among the top health IT-enabled primary care practices, a large amount of physicians are still doing care coordination the old fashioned way.
This was one of the core findings of new research published in the Annals of Family Medicine. Researchers, led by the National Committee for Quality Assurance (NCQA), examined how 350 primary care practices who had achieved patient centered medical home (PCMH) recognition would stack up against many of the Stage 3 requirements for care coordination.
